What makes concrete last here

• Compacted class 5 base, deep enough that frost does not lift the slab unevenly

Why concrete fails in Minnesota

Surface flaking (scaling) is almost always salt on young concrete, a mix without proper air entrainment, or water worked into the surface during finishing.
